Sabbatical Change Form
Faculty Liaisons聽in coordination with the faculty member should submit this form to request a sabbatical change.聽 If the sabbatical change request is a postponement of more than three semesters (that is, when it will not take place either within the same academic year that the sabbatical was to take place or the next academic year), this form should not be used and the faculty member should submit a new application for sabbatical.
