Immunization Requirements

Processing immunization records can take up to 10 business days. If you have not heard from the Immunization Compliance team after 10 business days, review your secure messages or 91福利社 email for information or follow-up requests.

Your document(s) must include all necessary information prior to uploading them into the portal. Vaccine documents must:

  • Include an official medical facility or government letterhead
  • Show the day, month and year of each required vaccine
  • Be in English or manually translated into English
  • Include your full name and date of birth

If your record(s) are missing any of these pieces of information, they will not be accepted for verification.

Processing immunization records can take up to 5 business days to review and 7-10 business days during peak submission periods (dates around immunization deadlines). If you have not heard from the Immunization Compliance Team after 5 business days, review your secure messages or 91福利社 email for any attempts of communication or requests for additional information.

If you have a registration hold and would like to request an expedited review of your account, please reach out to immunizations@colorado.edu.

If you are not in possession of your records, please try contacting your physician or state鈥檚 online registry to have a copy sent to you.

Colorado residents: Colorado immunization information is automatically uploaded to your student portal through the Colorado Immunizations Information System (CIIS), a confidential system that collects and consolidates vaccine data for Coloradoans.

Out-of-state students: Students with out-of-state records can request registry records through your student portal by following these steps:

  1. Log in to mycuhealth.colorado.edu using your CU IdentiKey and password.
  2. Select 鈥淕et Started鈥 under 鈥淪tudent Health Requirements.鈥
  3. Select option 鈥1. Enter Vaccination Date.鈥
  4. Select the yellow button 鈥淩equest My Out-Of-State Immunization Registry Records.鈥

Please note: Complete immunization information may not be uploaded to registries. If you cannot secure proper immunization records, you must complete one of the following:

  • Receive the MMR immunization and upload proof of vaccination.
  • Obtain MMR titers (proof of immunity) and upload a record of positive, quantitative results in place of an MMR vaccine record.
  • Complete and upload required exemption forms to opt out of the MMR requirement.

Meningococcal exemption: The Meningococcal Exemption Form can be signed and submitted directly through your MyCUHealth portal (no upload required).

  1. Log in to mycuhealth.colorado.edu using your CU IdentiKey and password.
  2. Select 鈥淕et Started鈥 under 鈥淪tudent Health Requirements.鈥
  3. Select option 鈥3. Complete Forms.鈥
  4. Use the fillable online form to complete the Meningococcal Disease Info and Vaccine Waiver.

MMR Exemption: You must complete one of the following options to waive the MMR immunization requirement:

  1. Certified Medical Exemption Form (must be signed by a provider)
  2. Certified Non-Medical Exemption Form (Must be signed by a licensed Colorado provider)
  3. 91福利社鈥檚 Online Immunization Education Module located on the 91福利社 Immunization Requirement website.

All MMR exemption forms must be filled out and uploaded to your MyCUHealth portal. If you complete the 91福利社 Exemption Module, you must upload your certificate of completion to your MyCUHealth portal.

To upload forms, please follow these steps:

  1. Log in to mycuhealth.colorado.edu using your CU IdentiKey and password.
